Separate Observations from Explanations

Begin with what happened: attendance, timing, equipment problems or participant feedback. Label interpretations as interpretations. This keeps the team from treating the first explanation as established fact.

Ask Three Questions

What should we repeat? What got in the way? What will we change next time? Give quieter team members space to contribute before the most confident voice settles the answer.

Record Only What Is Needed

Write the decision, owner and review date. Move safeguarding incidents, health details and personnel concerns into their proper confidential process. A general debrief log should not become an informal medical or HR record.
