Legacy North Academy Inc. Read these terms before completing registration. A parent or legal guardian signs for a participant under 19. Contact Ben with questions or to arrange a paper alternative.

1. Program and Signer

Operator: Legacy North Academy Inc., operating as Legacy North Academy (“Legacy North”). This registration is with the program operator, not the proposed Legacy Lives Forever foundation.

Your registration identifies the participant, age, selected program and session. The selected schedule provides the location, dates and times. These terms cover only that selection. Separate travel, overnight stays, off-site activities and other programs are not included.

For a participant under 19, the signer confirms that they are the participant’s parent or legal guardian and have authority to give this permission. An adult participant signs for themselves. A youth’s understanding and willingness to participate should also be discussed in language they understand.

2. Activities and Risks

Activities may include basketball instruction, drills, games, supervised active recreation and age-appropriate conditioning described for the selected program. Running, jumping, changes of direction, balls and contact with players, equipment or surfaces can cause falls, collisions and injury. Possible outcomes include cuts, sprains, fractures, concussion, other head or spinal injury, serious disability and, in rare cases, death. Exertion, dehydration, illness and individual health conditions can add risk. Food, where provided, can present allergy risks.

I understand that instruction, reasonable precautions and supervision cannot eliminate every risk. I have had an opportunity to ask questions about this program and choose voluntarily to participate or give permission for my child to participate with these risks understood.

3. Safety and Participation

The participant will follow reasonable safety directions, respect other people and equipment, and stop and tell a coach if injured, unwell, overwhelmed or unsafe. They may decline an activity or ask for an adjustment. Bullying, harassment and dangerous conduct are not acceptable; staff may pause participation and contact the guardian to address safety concerns.

I will privately share information reasonably needed for safe participation, such as activity restrictions, allergies, emergency medication arrangements or support needs, and update it when it changes. I will not put detailed medical histories in a general website message. This permission is not a medical clearance or a promise that the participant has no disability or support needs.

4. Emergency Assistance

If an urgent incident occurs, staff may provide first aid within their training, contact emergency services and share the information responders reasonably need. Staff will try to contact the guardian and emergency contact promptly. Healthcare professionals determine treatment and obtain consent where required. This does not authorize a coach to consent to any and all medical procedures. Registration does not promise that ambulance or medical expenses are insured or reimbursed.

5. Arrival, Departure and Communication

Check in with the program lead at the scheduled start time and arrange pickup at the scheduled finish. Do not leave a child before staff have checked them in. A guardian identifies authorized pickup people; independent departure requires an explicit guardian request and program approval. Staff are not authorized to transport the participant under this form. Keep contact numbers current and promptly tell the team about a change. Your completed checkout records your selected program and schedule. Independent departure and any individual arrangements still require confirmation from the program lead.

Registration-related messages may include confirmation, schedule changes, cancellations, safety information and attendance follow-up. Promotional newsletters require a separate optional signup.

6. Youth Consent—Participants Under 19

Required guardian acknowledgment:

“I have read the selected program information and Sections 1–5. I understand the described risks, have had the opportunity to ask questions and give permission for the named participant to attend. I have discussed safety expectations with them and confirm my authority to give this permission.”

This youth agreement records informed permission and responsibilities. It does not claim to extinguish the child’s legal rights or require a guardian to reimburse Legacy North for a claim arising from Legacy North’s own negligence.

7. Adult Release—Only for Participants 19 or Older

ADULT PARTICIPANTS: READ THIS SEPARATE RELEASE CAREFULLY. IT AFFECTS YOUR RIGHT TO CLAIM COMPENSATION, INCLUDING FOR NEGLIGENCE.

In exchange for being permitted to participate in the identified program, I accept the described risks and release Legacy North Academy Inc. and its directors, officers, employees, coaches and volunteers acting in connection with that program from claims I may have for injury, death or property loss arising from my participation, including claims caused by their negligence in instruction, supervision, equipment or premises. This release applies only to the extent permitted by law. It does not release intentional misconduct or rights that cannot lawfully be excluded. It is governed by British Columbia law, subject to mandatory rights and jurisdictional rules. If a provision is unenforceable, the remaining provisions apply to the extent permitted by law.

Separate required adult acknowledgment:

“I am 19 or older, have read and understood this adult release, understand that it includes negligence claims, have had the opportunity to ask questions and voluntarily agree to it.”

8. Photo and Video Permission

Your media choice: everyone must select Yes or No; either answer allows registration. Nothing is preselected.

Legacy North would like to photograph or film program activities to celebrate participants, explain its work and promote future opportunities. With permission, identifiable images, video and recorded voice from the selected program may appear on Legacy North’s website, its social channels, newsletters, printed program materials and its own sponsor or impact presentations. This permission does not let sponsors independently use a participant’s image in their advertising.

Images may be cropped or adjusted for colour and sound without misrepresenting the participant. Full names, personal stories or athlete interviews require separate permission. This permission does not cover changing-room photography, livestreaming, facial recognition, AI training, fabricated likenesses or sale of a participant’s image. There is no payment for approved use.

Public material may be copied or shared beyond Legacy North’s control. Contact ben@legacynorthacademy.com to change the choice or request withdrawal. Legacy North will stop new uses within a reasonable time and address removal requests for channels it controls; it cannot guarantee recovery of distributed print or third-party copies. Withdrawal does not prevent participation.

○ No—Please do not use identifiable photos, video or voice of this participant for these purposes.

Tell the program lead if your media choice changes. Staff use your recorded choice when planning photography and reviewing media. A child’s objection to being photographed is respected even where a guardian has given permission.

9. Information and Signing

Legacy North Academy Inc. uses registration details to administer your selected program, contact your family, record consent, support attendance and respond to emergencies. Ben Eby is your privacy contact: ben@legacynorthacademy.com. Authorized program staff access only the information they need. Squarespace hosts and stores the form, and our email provider processes registration notifications; their systems may process information outside Canada. Necessary information may be shared with emergency responders. Registration information is not a sponsor mailing list. We retain records for program administration, safety, consent and applicable legal or insurance needs, review retention periodically and securely remove information no longer needed. Contact Ben to request access, correction, or information about retention and service providers.

By submitting the applicable acknowledgment and your typed name, you intend to sign these terms electronically. The stored registration includes your selections, terms version and submission time. Keep your order confirmation and these terms. Contact Ben to request a copy of your registration record or a paper alternative. A completed order reserves the selected program place, subject to the published eligibility requirements; adding a registration to a cart alone does not complete enrollment.
