Keep a Decision Log with Reasons

A brief decision log prevents teams from repeatedly reopening the same issue because the reason was lost. Capture enough context for future leaders to understand the choice.

When to use it

Use this when decisions affect schedules, access, spending or recurring program practices.

Put It into Practice

  1. Record the decision, date, accountable owner, information considered and key constraint in plain language; link to the approved source rather than copying private records.
  2. State whether the decision is final, a trial or awaiting another approval, and name any condition that should trigger a review.
  3. Keep the log accessible to the appropriate team and revisit entries when circumstances change, documenting revisions without erasing the earlier reasoning.

What It Can Look Like

A program records that a session time was selected because it was the only confirmed venue slot, with a review planned when the next booking window opens.
